Reason #4 – Tax changes, HSA changes, and tax reporting of employers providing benefits. Need we say more? A PEO helps you with payroll and tax reporting, they will do this for all of their clients once it’s enacted, so why worry about it – it will be automatic.
Reason #5 – You will be more competitive then ever before. Quite honestly, there will be a number of employers that will throw their hands up and take the hit on penalties and other ramifications. 5 – BENEFICIARIES – Another far too common financial mistake is insurance beneficiary blunders. Please know that once a person passes away, the beneficiaries on their insurance policies are all but set in stone. Too many times insurance beneficiaries are not updated or changed with events that happen in life. People buy insurance, make a will, and lock it away in some fire proof safe or safety deposit box and forget about it until someone asks if you have one and you think, “Yes, I have that all taken care of.” You need to be sure to review and update these documents on a regular basis. One recent story that comes to mind, I was sitting in my office and received a phone call from a lawyer who was sitting in her office with a widow. She had just unexpectedly lost her husband of twelve years (and the father of her youngest child). Both the widow and her husband had been married once before. He had assured her that he did have insurance and that if anything were ever to happen to him, that she would be taken care of. However, the insurance policy that he did have in place was an old one (and was not a small one either) but,[link widoczny dla zalogowanych], since he had purchased the life insurance before his divorce from his first wife, or the birth of his youngest kid, and never had taken the time to update his policy, the policy still read that the first wife was the insurance beneficiary and all of the settlement went to her. 7 - EXIT STRATEGY – The seventh largest financial blind spot is the lack of a solid exit strategy. Many people put money into something because it is “a good buy” but, you don’t make money buying things, you make your profit selling them. It is good to get things for a low price but, only if you can sell them at a higher price. You should know what your exit strategy is from any purchase, savings, or investment that you make. You should always know what you are getting into but, more importantly you should know why you are getting into it and how, when,[link widoczny dla zalogowanych], and what you can expect to get out of it. Getting into any investment can be a mistake if you have not clearly defined your exit strategy. This can be said for your working career as well. Not everybody would feel comfortable retiring at 55, not everybody wants to work to 65. 6 - RETIREMENT PLANNING – I also see many mistakes that run in conjunction with retirement planning. Not saving enough, not saving soon enough, not placing retirement savings in the proper vehicles, not reallocating as your life and needs change. “Everybody has some money that they don’t want to lose!” Make sure that you are taking into account that current retirement savings is actually future retirement income. Most times this is money that you will rely on to maintain your standard of living. Make sure you consider what needs to happen, what your goals are,[link widoczny dla zalogowanych], and how these may change as you continue to save towards retirement. Risk and growth in the early stages of retirement saving should be toned down as we near retirement age, because it becomes harder to make up for losses. Losing 30-40% of your retirement savings the year or two before you plan on retiring can be devastating and can severely alter plans. The market is unpredictable. It is fine to have a gambling account, so long as you know and recognize that that is what it is. Because of great market returns in the late eighties and through the nineties many savers began to co-mingle their retirement savings with their gambling investment accounts in hopes of big returns and a better retirement nest egg. Unfortunately, we have seen three major market downturns since 1998, which had helped many of these same people see all too vividly the error in this logic. This can also happen after you are well into retirement. Now they have half the money and income they had when they entered retirement and are either severely altering their lifestyle or, are going back into the work force. “It is harder to gain money than it is to lose it.” A simple math problem… When does -50 plus 100 equal 0? When you are talking about the percentage of you money. If you lose 50% of your money, it takes a 100% gain to get back to break even. That is a 100% gain, just to make it back to your original starting point. When is the last 100% market or investment gain that you remember having? How about the last 40-50% drop? Use rules of investing like the rule of 100 to give you a starting point for proper allocation and diversification. Make sure to structure savings and investments so that you can never run out of income once you do retire and so that you are protected and safeguarded against inflation,[link widoczny dla zalogowanych], market fluctuation, and events that are eventualities in life such as the death or loss of a spouse.
2 – TOD – Similar to POD, TOD means Transferable On Death and is for your beneficiaries to gain access to needed funds from any type of brokerage account that you may hold. A TOD form should be included with any brokerage account to which you would like your loved ones to have access to upon your passing. The reason that you probably are unaware of these forms is that is the banker or broker you deal with either does not know that these are forms they should provide or, does not want to provide them to you. What happens in probate? Your money is tied up while disputes and claims are settled. Who keeps the money while those disputes are settled? The bank or brokerage house with which the account is held holds that money until probate is disputed and settled. Why would they want to keep control of money while it is in dispute? Because they continue to earn interest and returns off of the money while it is in dispute and until probate is resolved. This is money that could be needed and put to use or, put to work for your loved ones. Make sure that you have these important forms attached to your accounts.
4 - BUY/SELL PARTNERSHIP AGREEMENTS– In business,[link widoczny dla zalogowanych], the importance of having buy/sell partnership protection cannot be understated. If you share in the ownership of a business with one or more partners, and they were to pass away, how would you like to suddenly be dealing with the spouse or family of your business partner in the decision making process of your business? If no buy/sell protection is established,[link widoczny dla zalogowanych], this is how that transaction would progress. Does your partners’ spouse know about your business, how to run it, do they know how it operates, do they know how to budget or run the books, do they know the client’s needs and wants? Could they even recognize a client? Would they make the same decisions about the business that your partner would have made? If you do not have a buy/sell agreement in place, prepare to deal with this eventuality. Be sure to have a buy/sell partnership agreement in place so, that if something was to happen,[link widoczny dla zalogowanych], life insurance would kick in in an amount that would allow the remaining partner(s) to buy the surviving spouse’s portion of the business back at a fair price and that these wishes are properly discussed, agreed upon, and documented by all business owners. I reviewed the chart and saw that she had been on Zoloft in steadily increasing doses. She insisted the increasing doses had only made her feel worse. I tracked down more about her history,[link widoczny dla zalogowanych], including a list of all medications. I found out that while her Zoloft had been increasing at the mental health clinic, she had continued to take medication prescribed by her primary care physician at another clinic.
In any event, she called me or I called her every day. I just couldn’t figure out a more efficient way of doing this — although I did offer her hospitalization which she declined.
When I tried to speak to her physician, I discovered that he was a decent man. He had put her on Amitriptyline,[link widoczny dla zalogowanych], a tricyclic anti-depressant. He told her that she could get her medications “adjusted” at the mental health clinic.
Since no one on the staff had ever drawn a tricyclic level before, I talked until I was blue in the face. They had to send the tricyclic level out of state and told me I’d get results in about a week.
Increased depression and increased sedation are some of the less dangerous clinical side effects of toxicity. People also try to kill themselves using tricyclics, and this chronically suicidal woman told me she’d had that thought. And if a person takes a week’s worth of a tricyclic prescription at once, what doesn’t kill them could cause considerable cardiac conduction difficulties,[link widoczny dla zalogowanych], possible heart attack, and more.
And you guessed it –the other psychotherapists were angry at me. Such is my life. The patient got better and my job was done. I’m not in this to make friends and I’m not in this for ego. I’m in this to help people, plain and simple.
She’d recently had an electrocardiogram as part of a general annual physical and lucky for us, there were no signs of conduction reflex. This did not necessarily mean there was no toxicity at all. This meant that the toxicity had not been powerful enough to affect her cardiac function.
I suggested she go home and try skipping that evening’s dose of tricyclic. By doing this, there were two things that could happen. She could start feeling better, and this would be the fastest way to diminish her toxicity. Or she could start feeling quite ill. The sudden severance from tricyclics at a therapeutic level will often cause an intractable headache which is resistant to all basic antalgics — like aspirin or Tylenol — given for headache. Depending on how she felt, we could consider restarting her on a lower dose the next day.
Regardless of all this, her face was red and she was crying. She told me she was chronically suicidal and never thought about anything else. Despite being medicated, her depression seemed to have gotten worse.
Well, the mental health clinic did not realize she was already on Amitriptyline. So, she was still taking the Amitriptyline prescribed by her physician of origin while she was taking the Zoloft. There is an interaction between these two medications that is well documented. As her Zoloft increased, so did her Amitriptyline level.
Since we had a clear biochemical basis for her diagnosis and she was already getting better, I decided to go and see the director of the internship program. She was a seasoned therapist who became so angry at me that I probably should have ducked under the desk. She simply refused to believe that medication side effects alone could create the level of suicide data which she had seen. She insisted on continuing the psychodynamic therapy for what she thought was ego-dystonic lesbianism.

I can vouch for the fact that this woman was as ego-syntonic as ever I have seen. She looked as if she had been born to wear her cap and her tattoo and her jacket.
The next day, she had no symptoms whatsoever and she was starting to feel better. She said she wasn’t thinking about suicide, which was very good news. So I put her on a program of daily calls and monitored her dose.
In about a week, the tricyclic levels came back. They showed that her combined total of metabolites was about 50 percent more than we would have expected. This is nowhere near the worst case of tricyclic toxicity I’ve seen, but as in so many things, there is absolutely no correlation between the biochemical severity of the symptom and how the patient presents clinically.
In the meantime, I needed to help her feel better. She would have to get off one of the two drugs. It seemed to make more sense to continue Zoloft, which was safer to continue than the Amitriptyline. I knew that any toxic effects she had would not be from Zoloft, which has a therapeutic index that doesn’t really require blood levels. With Amitriptyline — which is a tricyclic – blood levels are required.
People can get into trouble with this class of drugs. In this class of anti-depressants — known as tricyclics — one of the first signs of toxicity is an increase in the intensity of the depression. I told her immediately I thought this was most likely her problem and sent her to the local laboratory for a STAT. That meant immediately, on emergency basis, to find her tricyclic level.
So we brought her through and we brought her out of it — but there is one curious side effect here. This woman had been a weekly psychotherapeutic patient for at least two,[link widoczny dla zalogowanych], maybe three different interims. She had been marked as someone whose problems were psychodynamic since anti-depressant drugs had failed her. Psychotherapeutic interns had been sent to the books to read up on and discuss with her the psychodynamics of lesbianism.
I enjoy having friends, like just about everyone does. But that’s not why I’m in this business. When a patient needs help,[link widoczny dla zalogowanych], I will do my best for them every single time. And if a few colleagues get bruised egos along the way, so be it.
It was back in my residency that I learned something from Dr. Sheldon Preskorn at the University of Kansas,[link widoczny dla zalogowanych], Wichita. He said that tricyclics simply cannot be prescribed safely without knowing blood levels. He also suggested there was no need for a physician to learn about the pharmacokinetic and pharmacodynamic effects of tricyclics. The effects of the toxicity were already well known.
I talked to this patient and told her that in the meantime, I would deal with her clinically. The health center I was working in made it so that I could not see her in less than one week, so we took some telephone appointments. Clinically,[link widoczny dla zalogowanych], she had the signs of toxicity –she was a little dizzy and a little woozy. The tricyclic medications can lead to diminution and alter static blood pressure, so all of her symptoms were consistent.

We’re talking about someone who was short and stout and wore the kind of cap one would expect to see on a newsboy during World War I. So why treat? We certainly have enough people who really need therapy and who could be helped by interns. But as with many situations, securing a patients’ diagnosis and removing the physical symptoms was not enough to get other people to abandon their belief systems. Belief, after all, is the strongest force in the universe. Even stronger than pharmacology, I think.
At any rate,[link widoczny dla zalogowanych], this woman became rededicated to the relationship with her partner, which had been hanging on by a thread. Later on,[link widoczny dla zalogowanych], she told me that it had blossomed. She asked me what she should do about the psychotherapists. I told her that her beliefs were different from mine and only she knew what was helping her and that she could make her own decisions. So she told me she didn’t think she needed to see them so often.
